Forms in the portal
If salon services have intake forms, customer fills them out in the portal.
Where in the portal
After login, the Forms section:
- Pending: tied to current or upcoming bookings
- Submitted: previously filled
When does customer get a form
During booking
Flow prompts. Customer fills before booking finalizes.
After booking
For some services, form arrives after booking.
On salon request
Manager can ask existing customer to fill "Updated allergy disclosure".
What customer sees
- Title (e.g., "Allergy disclosure")
- Intro text
- Questions in various types
- Submit button
After submit, manager-side displays it on customer record.
Use case scenarios
Scenario 1: Mandatory allergy disclosure
New customer books waxing. Flow prompts. Reception sees what they're allergic to at booking record.
Scenario 2: Hair style consultation
Hair color booking has "Hair history and preferences" form. Stylist knows what to do in advance.
Scenario 3: Post-booking health questionnaire
Customer fills in portal within 24 hours.
Scenario 4: Annual renewal
Yearly refreshed "Data processing consent".
Scenario 5: Medical consultation
Aesthetic clinic "Medical anamnesis". Doctor reviews before visit.
Tips
- Make form available before the visit.
- Mandatory forms in booking flow.
- Push notifications for pending forms.
- Submitted forms visible on manager side too.
- Form answers are part of anamnesis.
