If salon services have intake forms, customer fills them out in the portal.

Where in the portal

After login, the Forms section:

  • Pending: tied to current or upcoming bookings
  • Submitted: previously filled

When does customer get a form

During booking

Flow prompts. Customer fills before booking finalizes.

After booking

For some services, form arrives after booking.

On salon request

Manager can ask existing customer to fill "Updated allergy disclosure".

What customer sees

  • Title (e.g., "Allergy disclosure")
  • Intro text
  • Questions in various types
  • Submit button

After submit, manager-side displays it on customer record.
